Welcome To Our Practice
Here is some information that will be helpful if you will be seeing us for the first time, or in familiarizing yourself with our office procedures.

If you are a new patient to our practice, please download and complete the
 new patient forms 
and bring it with you to your first appointment.

Please come in 15 minutes before your scheduled time. If you need to cancel an appointment, please call 48 hours before if possible. Cancellations should be made no later than 24 hrs. prior to your appointment or you will be billed.


When you arrive, please give the receptionist your name. Your doctor will take your history and perform a physical examination, concentrating on your muscles, bones, and joints. He/She will discuss his/her thoughts about your problem and its treatment.

Should you need X-ray studies, you may have them done at our facility, provided your insurance carrier allows this. Should you need laboratory studies, our nurses and/or front desk receptionists will be happy to direct you to the appropriate facility. All test results will be given to you at the time of your return appointment. Should your tests need attention prior to your return, we will contact you.

For your first office visit, you will need to bring the following:

  • All of your medications
  • Copies of any laboratory, X-ray reports, original films or hospital reports done in the last year
  • All insurance cards

If we participate in your insurance plan, we will submit claims for you. All referrals and co-pays are due at the time of your visit. If you do not have your co-pay or referral, your appointment will need to be rescheduled. If we do not participate in your insurance plan, payment is expected at the time of service. We will submit covered laboratory and X-ray services to your insurance company for you. Any services deemed non-covered or deductibles are your responsibility.

Even though we may participate or are in-network with your insurance there is no guarantee their payment will be accepted as payment in full. Some plans, depending on the procedures billed, may have co-insurance and/or deductibles the patient is responsible for.
